We take a collaborative approach to community health improvement by routinely conducting Community Health Needs Assessments (CHNAs) in our service areas. During the CHNA process we listen to community leaders and members with diverse backgrounds, voices, and experiences to understand their unique health needs and the community-based assets.

Through the CHNA, we identify Significant Health Needs that we prioritize for the next three years.

To address Significant Health Needs, we develop an Implementation Strategy in collaboration with our communities that focuses on data-driven needs and solutions.
